The Fiscal Year (FY) 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality primary and preventive health care services in the United States and its territories.

Eligibility

The following entities are eligible for this grant:

• City or township governments
• Public housing authorities/Indian housing authorities
• County governments
• Special district governments
• State governments
• Native American tribal organizations (other than Federally recognized tribal governments)
• Nonprofits having a 501(c)(3) status with the IRS, other than institutions of higher education
• Native American tribal governments (Federally recognized)
• Nonprofits that do not have a 501(c)(3) status with the IRS, other than institutions of higher education

Applicants must propose to serve at least one medically underserved area or medically underserved population (MUA/MUP) within the proposed service area, which are listed in HRSA’s MUA/P tool.

This program has a two-phase submission process.

Phase 1: Applications must be submitted through the Grants.gov listing by October 19, 2026 at 11:59pm. which requires accounts for both Grants.gov and SAM.gov.
